§ 33-1-210 — Forms.
(a)The Secretary of State may prescribe and furnish on request forms for:
(1)an application for a certificate of existence, (2) a foreign corporation's application for a certificate of authority to transact business in this State, (3) a foreign corporation's application for a certificate of withdrawal, and (4) in conjunction with the Department of Revenue, the annual report. If the Secretary of State so requires, use of these forms is mandatory. The Secretary of State, through regulation, may prescribe a mandatory form in regard to any other forms required or permitted by Chapters 1 through 20 of this Title to be filed in his office. All such mandatory forms must comply with all statutory requirements contained in Chapters 1 through 20 of this Title.
