(1)To be effective, the notice referred to in s. 712.05, other than the summary notice and the amendment referred to in s. 712.05(2)(b), must contain:
(a)The name or description and mailing address of the claimant or the property owners’ association desiring to preserve any covenant or restriction.
(b)The name and mailing address of an owner, or the name and mailing address of the person in whose name the property is assessed on the last completed tax assessment roll of the county at the time of filing, who, for purpose of such notice, shall be deemed to be an owner; however, if a property owners’ association is filing the notice, the requirements of this paragraph may be satisfied by attaching to and recording with the notice an affidavit executed by the appropriate member of the board
