No person shall plug and abandon an oil or gas well except in accordance with a permit issued therefor by the Department of Environmental Protection pursuant to this section. An application for this permit shall be made on forms prescribed and supplied by the department and shall contain at least the following information: a. The name and address of the owner; b. The signature of the owner or his authorized agent and when an authorized agent signs an application it shall be accompanied by a certified copy of his appointment as an agent of the owner; c. The location of the well, as identified by the municipal tax map by lot and block; d. Designation of well by name and number; e. The total depth of the well to be plugged; f.
