2A:18-34. The clerk of the Superior Court, shall require that there be filed in his office a statement, signed by the clerk of the Special Civil Part in which the judgment was entered containing: a. The name of the court, b. The names of the parties to the action in which the judgment was rendered, c. The name of the attorney, if any, of the party in whose favor the judgment was rendered, d. The amount and date of the judgment and e. The date of issue and return of execution, if any, f. Which statement shall be accompanied by an affidavit of the party, his attorney or agent, in whose favor the judgment was rendered that, at the time of the filing of the statement a certain stated amount, not less than $10, was still due on the judgment.
