§ 49-55b — Form of notice of vessel lien.
A notice of vessel lien shall contain the following information:
(1)The registration number;
(2)the name of the vessel;
(3)the name of the manufacturer;
(4)the type of propulsion;
(5)the length of the vessel;
(6)a general description of the vessel;
(7)the name and last-known mailing address of the owner;
(8)the name and address of the lienor;
(9)the amount of the lien;
(10)the basis of the claim with dates; and (11) the place where the vessel is being held. This notice shall be in the following form: NOTICE OF VESSEL LIEN To all persons whom it may concern, a lien is claimed by me on the below-described vessel: Name of claimant .... amount of claim ....
