(A)An applicant for registration shall submit an application for registration to the Department of Consumer Affairs in a form prescribed by the department. An application filed pursuant to this section is a public record. The application must be in the name of a person and, except as otherwise provided in subsection (B), signed or otherwise authenticated by the applicant under penalty of perjury and state or contain:
(1)the name, date of birth, and place of birth of the applicant and:
(a)the address of his principal place of business;
(b)his work and mobile telephone numbers; and (c) any means of communicating with him electronically, including:
(i)facsimile number;
(ii)electronic mail address; and (iii) personal, business, or employer websites;
(2)the name of the applicant's busine
