(A)If the legislative authority of a municipal corporation adopts a resolution for the purposes provided in section306.55of the Revised Code and division (XX) of section5705.19of the Revised Code and certifies the resolution to the board of elections as a combined question, the question appearing on the ballot shall read:
"Shall the territory within the ______ (name of municipal corporation) be withdrawn from ______ (name of regional transit authority) and shall an additional tax be levied for the benefit of ______ (name of municipal corporation) ______ for the purpose of providing transportation services for the movement of persons within, from, or to the ______ (name of municipal corporation) at a rate not exceeding ______ mills for each one dollar of valuation, which amounts to ______
