Text
(A)Except as provided in division (D) of this section, an individual seeking a license to practice as an anesthesiologist assistant shall file with the state medical board a written application on a form prescribed and supplied by the board. The application shall include all of the following information:
(1)Evidence satisfactory to the board that the applicant is at least twenty-one years of age;
(2)Evidence satisfactory to the board that the applicant has successfully completed the training necessary to prepare individuals to practice as anesthesiologist assistants, as specified in section4760.031of the Revised Code;
(3)Evidence satisfactory to the board that the applicant holds current certification from the national commission for certification of anesthesiologist assistants and
