Text
(A)Except as provided in division (C) of this section, an individual seeking a license to practice as a physician assistant shall file with the state medical board a written application on a form prescribed and supplied by the board. The application shall include all of the following:
(1)The applicant's name, residential address, business address, if any, and social security number;
(2)Satisfactory proof that the applicant meets the age requirement specified in division (A)(1) of section4730.11of the Revised Code;
(3)Satisfactory proof that the applicant meets either the educational requirements specified in division (B)(1) or (2) of section4730.11of the Revised Code or the educational or other applicable requirements specified in division (C)(1), (2), or (3) of that section;
