(A)Except as provided in division (D) of this section, any person who desires to obtain a permit as a crematory operator shall apply to the board of embalmers and funeral directors on a form prescribed by the board. The applicant shall include with the application the initial permit fee set forth in section4717.07of the Revised Code and evidence, verified under oath and satisfactory to the board, that the applicant satisfies both of the following requirements:
(1)The applicant is at least eighteen years of age.
(2)The applicant has satisfactorily completed a crematory operation certification program approved by the board and has presented to the board a certificate showing completion of the program.
