Text
A dentist who desires to participate in the oral health access supervision program shall apply to the state dental board for an oral health access supervision permit. The application shall be under oath, on a form prescribed by the board in rules adopted under section4715.372of the Revised Code, and accompanied by an application fee of twenty-five dollars. To be eligible to receive the permit, an applicant shall meet the requirements established by the board in rules adopted under section4715.372of the Revised Code.
The state dental board shall issue an oral health access supervision permit to a dentist who is in good standing with the board and satisfies all of the requirements of this section.
Chapter 4796. of the Revised Code does not apply to a permit issued under this section.
