(A)Except as provided in division (C) of this section, the state cosmetology and barber board shall issue a practicing license to an applicant who satisfies all of the following applicable conditions:
(1)Is at least sixteen years of age;
(2)Has the equivalent of an Ohio public school tenth grade education;
(3)Has submitted a written application on a form furnished by the board that contains all of the following:
(a)The name of the individual and any other identifying information required by the board;
(b)A photocopy of the individual's current driver's license or other proof of legal residence;
(c)Proof that the individual is qualified to take the applicable examination as required by section4713.20of the Revised Code;
(d)An oath verifying that the information in the applica
