Section 15.23. TRANSFER OF AUTHORITY(a) A foreign business corporation authorized to transact business in the commonwealth that converts into a foreign nonprofit corporation or into any form of foreign other entity that is required to deliver for filing an application for authority to transact business in the commonwealth or make a similar type of delivery with the secretary of state if it transacts business in the commonwealth shall deliver to the secretary of state for filing an application for transfer of authority executed by any officer or other duly authorized representative. The application shall set forth:

(1)the name of the corporation;
(2)the type of other entity into which it has been converted and the jurisdiction whose laws govern its internal affairs;
(3)any other information
