Text
(a)The envelope required by section 28 of
this chapter must bear upon its face a printed affidavit containing the
voter's affirmation under penalties of perjury that the following
information is true:
The voter must indicate the voter's precinct and township (or ward
and city or town) and indicate whether the voter is entitled to vote
as a resident of the precinct or is entitled to vote under IC 3-10-11
or IC 3-10-12.
(b)The voter must sign and date the affidavit. After the voter has
signed and dated the affidavit, the absentee voter board members each
must sign the affidavit and print the member's name.
(c)The absentee voter board must indicate on the affidavit if:
(1)the board visited the voter in the manner authorized under
section 25(b) of this chapter; and
(2)the voter was a vote
