Text
(1)Individual and insurance
agency producer licensees shall inform the commissioner in writing, in a form
prescribed by the commissioner, of any change of address within thirty days after
the change.
(2)Failure of any licensee to inform the commissioner of any change to the
licensee's address of record or residence address shall be grounds for the
assessment of a penalty.
