§ 10-2-411 — Duplicate license
The commissioner may issue a duplicate license
to any actively licensed insurance producer if such producer's license is lost, stolen,
or destroyed upon an affidavit by the producer in a form prescribed and furnished
by the commissioner concerning the facts of such loss, theft, or destruction.
