(1)Upon request, every employer and employee shall furnish the division all
information required by it to accomplish the purposes of this article, which
information shall be furnished on blanks to be prepared by the division. It is the duty
of the division to furnish such blanks to the employer free of charge upon request
therefor. Every employer receiving from the division any blanks, with directions to
fill out same, shall answer fully and correctly all questions therein propounded and
give all the information therein sought, or, if unable to do so, he shall give in writing
good and sufficient reasons for the failure. The director may require that the
information required to be furnished be verified under oath and returned to the
