Each client admitted to the center shall have on file at the center an individual program plan. The individual program plans shall be prepared by an interdisciplinary team within thirty
(30)days of admission for residential services under the
supervision of the program manager. The plan shall be reviewed
by the interdisciplinary team for appropriateness and
feasibility of discharge or transition to another level of
service thirty (30) days after implementation of the plan, at
the end of each quarter for the first year and annually
thereafter.
