(a)The following statutory form of power of attorney to delegate parental or legal custody may be used: STATE OF WEST VIRGINIA STATUTORY FORM FOR POWER OF ATTORNEY TO DELEGATE PARENTAL OR LEGAL CUSTODIAN POWERS
