Text
Prior to disclosing personal information the division may require the person making the request to:
(a)Verify his or her identity;
(b)verify that the information will be used only as authorized, or that the consent of the person who is the subject of the information has been obtained; and (c) make and file a written application in such form and containing certification requirements as the division may prescribe.
