(1)The poll workers shall follow the procedures and requirements of this section when:
(1)(a) the individual's right to vote is challenged as provided in Section 20A-3a-803 or 20A-3a-805;
(1)(b) the individual's name is not found on the official register; or
(1)(c) the poll worker is not satisfied that the voter has provided valid voter identification.
(2)When faced with one of the circumstances described in Subsection (1)(a) or (b), the poll worker shall:
(2)(a) request that the individual provide valid voter identification; and
(2)(b) review the identification provided by the individual.
(3)If the poll worker is satisfied that the individual has provided valid voter identification that establishes the individual's identity and residence in the voting precinct:

(1) The poll workers shall follow the procedures and requirements of this section when:

(3)(a) the poll worker in charge of the official register shall:
(3)(a)(i) record in the official register the type of identification that established the individual's identity and place of residence;
(3)(a)(ii) record the provisional ballot envelope number in association with the name of the individual; and
(3)(a)(iii) direct the individual to sign the individual's name in the official register or pollbook; and
(3)(b) the poll worker having charge of the ballots shall:
(3)(b)(i) give the individual a provisional ballot; and
(3)(b)(ii) allow the individual to enter the voting booth.
(4) If the poll worker is not satisfied that the individual has provided valid voter identification that establishes the individual's identity and residence in the voting precinct:
(4)(a) the poll worker in charge of the official register shall:
(4)(a)(i) record in the official register that the voter did not provide valid voter identification;
(4)(a)(ii) record in the official register the type of identification that was provided by the individual, if any;
(4)(a)(iii) record the provisional ballot envelope number in association with the name of the individual; and
(4)(a)(iv) direct the individual to sign the individual's name in the official register or pollbook; and
(4)(b) the poll worker having charge of the ballots shall:
(4)(b)(i) give the individual a provisional ballot; and
(4)(b)(ii) allow the individual to enter the voting booth.
(5) When, at a polling place, the election officer is required to furnish more than one version of a ballot, the poll workers at that polling place shall give the registered voter the version of the ballot that the voter is qualified to vote.
