Text

(1)As used in this section, "qualifying form" means:
(1)(a) a driver license application form; or
(1)(b) a state identification card application form.
(2)The lieutenant governor and the Driver License Division shall design each qualifying form to include:
(2)(a) (2)(a)(i) the following question, which an applicant is required to answer: "Do you authorize the use of information in this form for voter registration purposes? YES____ NO____"; and
(2)(a)(ii) no later than November 5, 2025, the following:"Indicate below how you want to vote in upcoming elections:_____ Mail a ballot to me._____ Do not mail a ballot to me. I will vote in person.";
