§ 23-19.12-9. Application for permit.
Application for a permit shall be made to the director of the department of environmental
            management upon a form prescribed by the director. Upon approval of the application
            by the director, a permit shall be issued. Permits shall expire three (3) years from
            the date of issue, unless sooner if suspended or revoked. The director of the department
            of environmental management is empowered to charge an annual application fee for a
            medical waste transporters permit.
