Text
The form of the ballot shall be: "For change of name of .......... to ............................... [ ] Against change of name of ....... to .............................. [ ]." If a majority of the votes cast favor changing the name of the municipality, the name shall be changed and the municipal clerk shall certify the results of the election and the new name which was adopted. One copy of the certification shall be retained by the municipal clerk, one copy shall be filed with the county clerk and one copy shall be filed with the secretary of state.
