Text
1.An individual seeking a license, registration, permit, credential, or endorsement related
to the practice of dentistry, shall apply to the executive director on forms prescribed by
the board.
2.The application must:
a.Be verified under oath that all statements contained in the application are true to
the applicant's knowledge;
b.Contain a recent autographed picture of the applicant; and
c.Include the application fee established by the board.
3.The board may, by rule, create or modify fees associated with any license, registration,
permit, credential, or renewal. A fee created or modified under this section is not a new
fee for purposes of subsection 7 of section 54-35-27.
