Text
A licensee shall renew the license annually pursuant to board rules. A licensee who fails to
renew the license on or before the expiration date may not practice as a physical therapist or
physical therapist assistant in this state, and may be subject to a late renewal fee. A licensee
shall report to the board a name change and other changes in contact information within thirty
days of the date of change.
