Each individual licensed or registered by the board shall provide information requested by
the board at the time of renewal or reactivation. Each individual licensed or registered by the
board shall report to the board any knowledge of the performance of those acts or omissions
that are violations of this chapter or grounds for disciplinary action as set forth in section
