Text
The director shall file and maintain each application for a license with suitable indexes containing:
1.All applications denied and on each denied application note the reason for the denial;
2.All applications granted; and
3.The name of every licensee whose operator's license has been suspended, revoked,
canceled, or restricted by the department and after each name state the reasons for
such actions.
