Text

1.  Any person in the person's clinical fellowship shall hold a provisional license to practice speech-language pathology or audiology.  The board may issue a provisional license to an applicant who:

(1)Has met the requirements for practicum and academic requirements from an accredited training program under this chapter;
(2)Submits an application to the board on a form prescribed by the board.  Such form shall include a plan for the content and supervision of the clinical fellowship, as well as evidence of good moral and ethical character; and
(3)Submits to the board an application fee, as set by the board, for the provisional license. 2.  A provisional license is effective for one year and may be extended for an additional twelve months only for purposes of completing the postg
