YOUTH EMPLOYMENT STANDARDS ACT (EXCERPT) Act 90 of 1978 409.104d Employer submission requirements to registration system before hiring minor. Sec. 4d. If an employer proposing to hire a minor receives notification via the registration system that the minor has registered for employment with the employer, the employer shall submit all of the following to the registration system before hiring the minor:

(a)The name and contact information of the employer.
(b)The name and contact information of the minor.
(c)The address of each of the employer's work sites.
(d)The address of the proposed primary work site of the minor.
(e)The name of each minor the employer is currently employing.
(f)A signed statement made under the penalty of perjury that the employer is or will employ minors in accor
