Text
An application for registration must be in writing and under oath to the administrator, in such form as the administrator may prescribe. The application must include the following information:
1.Name and residence.
The legal name and residence and business addresses of the applicant, if the applicant is a natural person, or, if the applicant is a partnership, association, or corporation, the name of every partner, officer, or administrator of the applicant;
2.Location.
The location of the principal office of the applicant;
3.Other locations.
The complete address of any other locations at which the applicant proposes to engage in the activities regulated by this subchapter; and
4.Other information.
