§711.13. Application for licensure or certification; fees
A. Application for licensure as a professional geoscientist shall be on forms
prescribed and furnished by the board, shall show the applicant's education and a detailed
summary of his technical work, and shall contain such references prescribed by the board.
Forms provided by the board for application shall contain the explanation for fees listed in
Subsections E and F of this Section.
B. Application for certification as a geoscientist-in-training shall be on forms
prescribed and furnished by the board, shall show the applicant's education and a detailed
summary of his technical work, if any, and shall contain such references prescribed by the
board. Forms provided by the board for application shall contain the explanation for fees
listed in Subsections E and F of this Section.
C. All applications shall include a statement affirming that the information contained
on the application is true and correct to the best of the applicant's knowledge, information,
and belief. Electronic signatures are acceptable, if properly encrypted.
D. The application fee shall accompany the application. If the board denies the
issuance of a license or certification to any applicant, the application fee shall be retained by
the board. An unsuccessful applicant shall pay the prescribed fee for each subsequent
application
E. The fees for professional geoscientists are as follows:

(1) Application/initial licensing fee $250
(2) Annual license renewal $200
(3) Annual license renewal for
licensees sixty years or older $125
(4) Late renewal penalty $100
(5) Temporary license (ninety days) $250
(6) Duplicate wall license certificate $35
(7) Affidavit of licensure $25
(8) Verification of licensure $25
(9) Insufficient funds fee $35
F. The fees for geoscientists-in-training are as follows:

(2) Annual renewal fee $100
