§ 37:691 — Records and reports

A.

(1)The board shall keep a record of its proceedings and a file of all applications, which file shall show:
(a)The name, age, and residence of each applicant.
(b)The date of the application.
(c)The place of business of such applicant.
(d)Educational and other qualifications of the applicant.
(e)Whether or not an examination was required.
(f)Whether the applicant was rejected.
(g)Whether a license or certification was granted.
(h)The date of the action of the board.
(i)Such other information which may be deemed necessary by the board.
(2)The records of the board shall be prima facie evidence of the proceedings of the board set forth therein, and a transcript thereof, duly certified by the secretary of the board, under seal, shall be admissible in evidence with the same force and
