Text
(a)All applications under this chapter for
a certificate to operate motor vehicles, intrastate or interstate, shall be
made on forms prescribed by the department.
(b)All applications for a certificate to operate motor vehicles
intrastate on the public highways shall be accompanied by a filing fee
of one hundred dollars ($100). Each petition for reinstatement of a
certificate to operate motor vehicles intrastate, on the highways of this
state, shall be accompanied by a filing fee of fifty dollars ($50). A
common carrier reinstatement shall only occur within two (2) years of
the voluntarily ceased operations date provided to the department by
the carrier.
(c)All applications for an emergency temporary certificate to
operate motor vehicles on the highways of this state in intrastate
