Text

An applicant for a license or an operating agent contract under this article must provide the following information to the commission:

(1)The name, business address, and business telephone number of the applicant.
(2)An identification of the applicant.
(3)The following information for an applicant that is not an individual:
(A)The state of incorporation or registration.
(B)The names of all corporate officers.
(C)The identity of the following:
(i)Any person in which the applicant has an equity interest of at least one percent (1%) of all shares. The identification must include the state of incorporation or registration if applicable. However, an applicant that has a pending registration statement filed with the Securities and Exchange Commission is not required to provide information
