(a)Whenever a licensee:
(1)closes an existing branch; or
(2)intends to open a new branch or relocate an existing branch; the licensee shall give written notice to the department. A licensee shall give written notice to the department of the closing of an existing branch under subdivision (1) not later than thirty (30) days after the date of the closing of the branch. Not later than thirty (30) days before the intended relocation or addition of one (1) or more branches under subdivision (2), the licensee shall provide to the department the written notice of its intention required by this section and shall request approval in a form prescribed by the director to add or relocate one (1) or more branches.
