The county board of each county which has appointed a board pursuant to this Division may, by resolution abolish such board provided such resolution also provides that:
(a)in counties which have established a county or multiple-county health department in accordance with Division 5-25 or its predecessor and have an existing Board of Health:
(1)The membership of the Board of Health in single counties be increased to 11 in counties where present membership is 8 and to 15 in counties where present membership is 12, a majority of which shall be members of the general public, (2) The employees, records, assets and liabilities of the board be transferred and assumed by the Board of Health, and (3) an additional tax be imposed by the county board at a rate, which shall not be increased at any t
