Text
1.Requests for child abuse information shall be in writing on forms prescribed by the
department, except as otherwise provided by subsection 2. Request forms shall require
information sufficient to demonstrate authorized access.

a.Requests for child abuse information may be made orally by telephone where a
person making such a request believes that the information is needed immediately and
where information sufficient to demonstrate authorized access is provided. In the event that

a request is made orally by telephone, a written request form shall nevertheless be filed
within seventy-two hours.
