(1)At the election under s. 567.01, the ballot used shall be printed on one side of a plain white piece of paper in the form following: OFFICIAL BALLOT NO. OFFICIAL BALLOT NO. OFFICIAL ELECTION BALLOT (Month) , (Day) , (Year) PRECINCT NUMBER County, Florida INSTRUCTIONS: Local Option Election on TWO QUESTIONS: QUESTION NUMBER 1 is to decide whether the sale of intoxicating liquors, wines, or beer containing more than 6.243 percent of alcohol by volume shall be prohibited or permitted in County, Florida.
