§ 35-9-111 — Device registration - application - fees - expiration - rules
(1)Each applicant for registration of a device shall file with the commissioner, in the
form and manner the commissioner shall designate:
(a)The name and address of the applicant and, if different, the name and
address of the person whose name will appear on the device;
(b)The name of the device;
(c)A complete copy of the labeling accompanying the device, including its
packaging, directions, and precautions for use, and a statement of all claims being
made for the device;
(d)If requested by the commissioner, a complete description of all tests
made and the results thereof, including, but not limited to, efficiency and hazard
data upon which the claims are based; and
(e)Any other information required by the commissioner.
