A
public or private entity, including a nonprofit organization, that facilitates the
exchange of health information among emergency medical service providers,
doctors, hospitals, nursing homes, pharmacies, home health agencies, health plans,
and local health information agencies through the use of health information
technology may facilitate the voluntary, secure, and confidential exchange of forms
containing advanced directives regarding a person's acceptance or rejection of life-sustaining medical or surgical treatment.
