(1)As soon as
practicable after the approval of each comprehensive, statewide public health
improvement plan pursuant to section 25-1-504, each county or district public
health agency shall prepare a county or district public health plan, referred to in
this section as the local plan. Each local plan shall not be inconsistent with the
comprehensive, statewide public health improvement plan required under section
25-1-504.
(2)Each local plan shall, at a minimum:
(a)Examine data about health status and risk factors in the local community;
(b)Assess the capacity and performance of the county or district public
health system;
(c)Identify goals and strategies for improving the health of the local
community;
(d)Describe how representatives of the local community develop and
