§ 17-104-303 — License validity

(a)A perfusionist's license is valid for two (2) years from the date it is issued and may be renewed.

(b)A person may renew an unexpired license by submitting proof satisfactory to the Department of Health of compliance with the continuing professional education and clinical activities requirements prescribed by the State Board of Health and by paying the required renewal fee to the board before the expiration date of the license.

(c)A person whose license has been expired for less than ninety (90) days may renew the license by submitting proof satisfactory to the department of compliance with the continuing professional education and clinical activities requirements prescribed by the board and by paying the required renewal fee and a penalty as established by the board.

(f) The department may renew an expired license without reexamination if the person was originally licensed in the State of Arkansas and at the time of the request for relicensure in Arkansas is licensed in another state and has been licensed to practice in that state for the preceding two (2) years before the request. The fee for this license shall be determined by the board. (g) The department shall notify each licensee in writing of the expiration date of the license at the licensee's last known address according to the records of the department.
