Text
(a)Participants may be required to provide access to the
following information, the collection and maintenance of which
by the program team shall be in a standardized format pursuant
to rules and regulations of the supreme court:
(i)Gender, race, ethnicity, marital status and child
custody and support obligations;
(ii)Criminal history;
(iii)Substance use disorder history, including
medical records and prior treatment;
(iv)Employment, education and income history;
(v)Number and health of children born to
participants;
(vi)Incidents of recidivism occurring before, during
and after successful completion of a program, or failed
participation in a program;
(vii)Mental illness history, including medical
records and prior treatment.
