(a)The application for a commercial driver’s license or commercial learner’s permit shall include the following:
(1)The full name and current mailing and residential address of the applicant.
(2)A physical description of the applicant, including sex, height, and weight.
(3)The applicant’s date of birth and proof of age.
(4)The applicant’s Social Security number, unless the application is for a nondomiciled commercial driver’s license or a nondomiciled commercial learner’s permit. The Commissioner must verify the name, date of birth, and Social Security number provided by the applicant with the information on file with the Social Security Administration.
