§ 32B-11-604 — Application for local industry representative license.
Utah·Title 32B Alcoholic Beverage Control Act·Ch. 32B-11 Manufacturing and Related Licenses Act·Part 32B-11-6 Local Industry Representative License Act
(1)To obtain a local industry representative license, a person shall submit to the department:
(1)(a) a written application in a form prescribed by the department;
(1)(b) a nonrefundable $75 application fee;
(1)(c) an initial license fee of $125, which is refundable if a local industry representative license is not issued;
(1)(d) verification that the person is:
(1)(d)(i) a resident of Utah;
(1)(d)(ii) a Utah partnership;
(1)(d)(iii) a Utah corporation; or
(1)(d)(iv) a Utah limited liability company;
(1)(e) an affidavit stating the name and address of any manufacturer, supplier, or importer the person will represent;
(1)(f) a signed consent form stating that the local industry representative will permit any authorized representative of the commission, department, or any law enforcement o
