Sec. 604.2011. SURRENDER OF CERTIFICATE OR TEMPORARY PERMIT.
(a)The advisory board may accept the voluntary surrender of a certificate or temporary permit. A person who has surrendered a certificate or temporary permit may not engage in activities that require a certificate or permit, and the advisory board may not return the certificate or permit to the person, until the person demonstrates to the satisfaction of the advisory board that the person is able to resume practice as a respiratory care practitioner.
(b)The advisory board shall by rule establish guidelines for determining when a person is competent to resume practice as a respiratory care practitioner.
