IF (1) (A) a person is an officer or employee of the department, OR (B) a person is an officer or employee of a licensee of the department, AND (2) (A) the person or the person's spouse, parent, grandparent, brother, sister, or child has an ownership interest in a residential facility that is not publicly held or an ownership interest in a business that is not publicly held that owns or manages a residential facility that provides mental health services or supports, OR (B) the person or combination of persons named in subdivision (2)(A) has an ownership interest of at least thirty-five percent (35%) in a residential facility that is publicly held that provides mental health services, OR (C) the person or combination of persons named in subdivision (2)(A), has an ownership interest of at le
