If the petition is for an increase in the number of commissioners, the proposition shall be submitted by separate ballot in the following form: Ballot on Increase in Number of County Commissioners and Commissioners' Districts (Place x or  in square at left of your choice). G For five commissioners. G Against five commissioners.
