(A)Application for a license as a real estate salesperson shall be made to the superintendent of real estate on forms furnished by the superintendent and signed by the applicant. The application shall be in the form prescribed by the superintendent and shall contain such information as is required by this chapter and the rules of the Ohio real estate commission. The application shall include the address of the applicant's current residence. The superintendent shall retain the applicant's current residence address in a separate record that does not constitute a public record for purposes of section149.43of the Revised Code.
